2015 Questionnaire

To enable the creation of the Reunion Book, please download and complete the questionnaire f The questionnaire cannot be completed online as there is no facility to save the information. The preferred option is: after downloading and saving the questionnaire on your computer, type in your responses to the questions and then email the completed questionnaire to  If you do not have internet access, or are not comfortable using the computer to complete the questionnaire, please contact Sue who will forward a hardcopy of the questionnaire to you.

We also require two photos, one from 1975 [or earlier] and a current photo. If you can, please scan and then email the scanned photo in JPEG (.jpg) format with the questionnaire to Sue. Alternatively, mail both the questionnaire and photos to Sue at 8 Helenslea Road, North Caulfield, 3161, or drop them in.
